Clause Shelf is a folder desk for three kinds of paper: an auto claim, a benefit statement, and a certificate someone asked you to send. Open the page that matches the sheet in front of you, copy the fields, and stop.

Start in Auto Claim Files when the shop invoice, estimate, or declaration page is the problem. Use Benefit Statement Files when the page is a 401(k) statement, an EOB, or an HSA receipt. Open Certificate Pages when a contract asked for a certificate and the boxes do not match.

We are not an insurer or a broker. A number on a page is an illustrative example unless the page is telling you to copy it off your own form.
